Level Up Hub is an internal application used by Level Up Resources to coordinate care for the people we serve. It is not a consumer product, and nothing on this domain is advertising.
The Hub holds health and contact information about the people our navigators screen and enroll — including information that is protected health information under HIPAA. We hold it to deliver services and to bill for them, and for no other purpose. We do not sell it, rent it, or share it for advertising.
Access requires an account created by an administrator; there is no self-signup. What each account can see is limited by its role, checked on the server on every request, and staff accounts can add two-step verification to their sign-in. Every change to a record is written to an audit log that names the account that made it.
This page and the front page of leveluphub.org are static. They set no cookies, load no third-party scripts, and run no analytics or advertising trackers. The application at app.leveluphub.org sets a single session cookie once you sign in, which exists to keep you signed in and expires on its own.
We use a staff member's email address only to operate their account: to send a password reset they asked for, an invite to an account an administrator created for them, or a notice that something changed on their account. We send no marketing email and keep no mailing list.
If a message hard-bounces, or if anyone reports one as spam, that address is recorded and we stop sending to it. Mail from this domain is authenticated with SPF and DKIM and is delivered over TLS.
To ask what we hold about you, to correct it, or to tell us a message reached you in error, call (929) 865-1020. If you are a patient, you can also ask any navigator you have spoken with.
